Following a review of the use of the asbestos license application form and discussion with the ALC Licensing Sub-Group, the forms below have been revised. Though the changes are relatively minor, you can download the updated forms below:

The revision is that the evidence document/information we require is now listed in the pre-populated evidence table and if an applicant doesn’t use a particular document requested they should record that and briefly explain. For example, the use of SOPs is not a statutory requirement and may not be part of the operation of a business. Noting that those documents have not been submitted will allow HSE to be clear that a document is not present, rather than being missed off the submission of evidence.
The revision is that guidance is now included for Section 5 and increased stress on how to submit evidence e.g. - clearly name and reference documents and input the Section number in the email title.
The new format documents will be used for those licenses that expire from November onwards.
